Please bring the following items with you to your appointment:

2. Insurance card

3. All medication bottles and dosages

4. Unless arranged ahead of time, you must bring your child for appointments.  If you need to speak with the doctor confidentially first, please notify the staff upon your arrival.

5. An interpreter if your primary language is not English 
(the interpreter must be a responsible adult)

6. Form of payment for any insurance copay or deductible (Check, Cash, Visa, Mastercard, Discover).

7.  Copy of referral from your family doctor or provider 
(if applicable)

8.  Any previous medical records, MRIs, CTs, EEGs, and relevant laboratory test results. Please bring any reports or notes from your family doctor or provider.

9.  School records if applicable

APPOINTMENT CANCELLATION/RESCHEDULE POLICY
Please note 24 hour cancellation notice is required to avoid penalty fees charged to you directly. Exceptions are made with documented illness of the patient. Therefore, please cancel or reschedule your appointment if you or your child is ill (e.g., cold, rashes or flu).

SECOND OPINIONS
In most situations, second opinions are single one-time consultations unless specifically excepted by staff and physician(s) of Child Neurology Specialists.  All second opinions require new patient intake forms signed and previous records brought to the appointment.
